This is my own personal Reiki Session Summary pdf that I use to notate during my remote Reiki sessions. There is a place to list goals for the session, to note findings from a scan of each chakra, past/present/future insights, as well as general notes. This form helps practitioners stay focused and organized, and provide the client with a summary of their session.
